First Visit
We always love welcoming new patients to our Woodbridge family practice. To save you some time at your first visit, we invite you to download our patient intake forms and fill them out in advance. With your consent we will also acquire your dental records, including x-rays and charts, to provide you with the most thorough level of care.
In advance of your first visit to Village Dental, we ask new patients to:
- Arrive 15 mins before your appointment time.
- Bring your dental insurance information.
- Be ready to provide information such as allergies and any medications you take.
Payment Policy
To make payment as quick and easy as possible, Village Dental Centre accepts debit and all major credit cards including American Express. We also offer interest-free personalized payment plans for treatments.
Insurance
We do not offer third-party payment at Village Dental Centre. Patients pay upfront for services and dental claims are submitted electronically.
Cancellation Policy
Life is busy, and we know that schedules change and emergencies arise. In the event that you need to cancel a scheduled appointment with us, we ask that you provide 2 business days’ notice of cancellation by telephone. This ensures that we can continue offering dedicated appointment times that fit your busy life.
